Self-Care and Independence

7% of the CNA exam · blueprint NNAAP Written (Oral) Examination Content Outline, effective April 2024

Short answer

Four of the 60 scored items. This is the restorative side of the role: training the resident in self-care within their abilities, assistive devices for transferring, ambulation, eating and dressing, maintenance of range of motion, proper turning and positioning, bowel and bladder training, and the care and use of prosthetic and orthotic devices.

Why it matters on this exam

Four items, but it carries a principle that decides answers across the whole exam: do for the resident only what the resident cannot do for themselves. Promoting residents' independence is one of the five areas required before any direct resident contact, at 483.152(b)(1)(iv).

It also maps onto two NNAAP skills — modified passive range of motion for one knee and ankle, and for one shoulder — so it is tested practically as well.

Key concepts

Section 483.152(b)(6), basic restorative services, names the content:

Common mistakes

Worked example

A resident can wash their own face but not their back. The correct approach is to set up the basin, hand them the washcloth, let them wash what they can, and assist only with what they cannot reach. Doing the whole bath would be faster and would be marked wrong: 483.152(b)(6)(i) is explicit that restorative care means training the resident in self care according to the resident's abilities, and independence maintained is the clinical goal.

Original practice questions

Self-Care and IndependenceDifficulty 3/5

When performing passive range of motion, how far should a joint be moved?

  1. ASlightly past the point of resistance to improve flexibility
  2. BThrough the full normal range regardless of resistance felt
  3. COnly as far as the resident can move the joint unaided
  4. DTo the point of resistance, and never into pain
Show answer and rationale

Correct: D. Resistance is the stopping point, and pain means stop and report. This is why the NNAAP skill has the aide instruct the resident to report any pain before the exercise begins.

  • A — Forcing past resistance risks injury to the joint and surrounding tissue.
  • B — Ignoring resistance to reach a textbook range is how injuries occur.
  • C — If the resident could move it unaided, the exercise would be active, not passive.
Self-Care and IndependenceDifficulty 2/5

A resident can wash their own face but cannot reach their back. What should the aide do?

  1. ALet the resident wash what they can and assist only with the rest
  2. BComplete the entire wash for the resident to save them the effort
  3. CLeave the basin and let the resident manage the whole wash alone
  4. DAsk the licensed nurse to decide how much help to provide today
Show answer and rationale

Correct: A. Section 483.152(b)(6)(i) requires training the resident in self care according to their abilities, and promoting independence is among the content required before any direct resident contact.

  • B — Doing everything is quicker and removes the independence the role exists to preserve.
  • C — Leaving the resident to manage what they cannot reach fails to meet a care need.
  • D — The aide can judge what the resident can do; escalating every decision is not the role.
